Alma College Driver Approval Request

Alma College requires all faculty, staff, students and volunteers that will be driving a college vehicle to complete a motor vehicle driving history check. Faculty, staff and volunteers must have a re-check every two years and students must have a re-check once per year.

I am aware that motor vehicle reports may be obtained as part of the evaluation of my job application and/or employment, and/or student experience at Alma College. I have completed this form and certify that the information provided is correct.

By submitting this form, I hereby provide my authorization for Alma College or its representatives to obtain such information to evaluate my driving record and to determine my qualifications to operate a College owned, leased, or rented vehicle as a representative of Alma College.
