Excellence in Inclusion Nomination Form

The Excellence in Inclusion Award is awarded to a faculty member, staff member, student, and student organization who are doing outstanding work in any of the following areas:

• Celebrating and valuing the varied identities, backgrounds, experiences, and perspectives in our varies communities (campus, locally, regionally, nationally)
• Recognizing and removing barriers to success in their communities (campus, locally, regionally, nationally)
• Providing equitable access to resources through education and advocacy
